Indonesia visa application form. Completed application form for Indonesia will be prepared within 5 business hours of order submission, and emailed to you to download, print and sign.
Original, signed United Kingdom passport. Passport must have at least 6 months remaining validity on the date of travel and have at least 1 visa page clear of any markings.
Passport Photo: 1 Include a passport style photo, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a surcharge associated with this service.
Proof of Status. Original ILR card or other proof of resident status in the UK, this should be valid for a minimum of six months upon date of entry to Indonesia.
Itinerary. Copy of round trip tickets or confirmed itinerary.
Yellow Fever Vaccination.
If the applicant has traveled within the last 90 days to any of the Yellow Fever Countries
an International Certificate of Vaccination for Yellow Fever will be required.
Authorisation from Indoneseian Immigration Office. The applicant must provide a written approval from the immigration office in Indonesia if they require multiple entries or if they wish to stay in Indonesia longer than 60 days.
Bank Statement. Copy of a recent bank statement showing proof of sufficient funds.
Employment Letter. An original letter from your employer/school (on business letterhead, with contact details), stating that a leave of absence has been granted and that you will be returning to your current job. The Employment Letter must be addressed to Indonesia High Commission If you are self-employed, include a copy of your business license and tax return. If you are retired please submit proof of your retirement fund.
Hotel Reservations. Copy of confirmed hotel reservations including name of the applicant, name and address of the hotel, duration of stay.
Additional Documents for Children.
- Birth Certificate. A NOTARIZED copy of the applicant's birth certificate.
- Letter of Consent. A NOTARIZED letter of consent signed by both parents, specifying who will be travelling with the child and authorizing the Consulate to issue a visa to the child.
